Talazoparib is a PARP inhibitor whose cardiac electrophysiological profile presents a literature discordance. In a prospective phase 1 pharmacokinetic-pharmacodynamic study in 37 cancer patients receiving 1 mg daily, mean QTcF changes ranged from -3.5 to 6.9 ms without concentration-dependent repolarization delay or morphologic abnormalities. However, a review in The Lancet Oncology categorized talazoparib as having an intermediate risk of QT prolongation without referencing specific primary data. Baseline ECG assessment and standard clinical vigilance are advised.
