Active Ingredient: MICAFUNGINE
Micafungin is an echinocandin antifungal targeting beta-1,3-D-glucan synthesis, generally considered to possess a low intrinsic arrhythmogenic risk and lacking significant CYP450 inhibition. However, the first clinical case report of polymorphic ventricular tachycardia resembling Torsades de Pointes was documented following a switch from fluconazole in a patient concurrently treated with amiodarone (post-event QTc at 480 ms). While direct hERG inhibition is not established, and there is only one case report, clinicians should exercise vigilance when combining micafungin with other QT-prolonging drugs, particularly in patients with baseline hypokalemia or structural heart disease.
