However : at high doses (70–1600 mg/day) it's highly cardiotoxic. Loperamide exerts a dual electrophysiological threat: - Potent hERG blockade (IKr): Leading to marked QTc prolongation and Torsades de Pointes (TdP). - Sodium channel inhibition (Nav1.5): Causing QRS widening and Brugada-like patterns. - Sudden cardiac death and malignant arrhythmias have been widely documented in the context of abuse.
