Clinical Description (Database Summary)

Fluconazole directly inhibits the IKr potassium current (IC50 = 48.2 uM / 14.8 ug/ml) and alters hERG channel trafficking at clinically achievable plasma concentrations. Clinical data indicate a dose-dependent effect on cardiac repolarization: while 400 mg daily did not show significant QTc changes, high doses of 800 mg daily prolonged the QTc interval by an average of 13.4 ms. Numerous cases of Torsades de Pointes (TdP) have been documented, predominantly in female patients, including recurrences in monotherapy. Strict ECG and electrolyte monitoring is recommended, particularly with high-dose regimens.
