Although pharmacovigilance databases (FAERS, JADER, CVARD) have identified a safety signal for QT prolongation with caspofungin, in vivo studies suggest this may be primarily driven by the underlying severe fungal infection or systemic sepsis rather than direct drug toxicity. However, caspofungin frequently induces hypokalemia, a major precipitating factor for Torsades de Pointes. Therefore, strict electrolyte and ECG monitoring remains highly recommended during therapy.
